HOW DO I KNOW WHAT TYPE OF SCREENING I NEED?

More often than not, your company/DER/consortium or any ordering official will either email you a confirmation page, or give you a hard copy in hand to bring with you when you come for your visit. It is imperative that we know the exact details of what kind of screening is required. Without this information we will be unable to fulfill your request.


WHAT DO I NEED TO BRING TO MY APPOINTMENT?

The first thing you will need to bring to the appointment is a valid for of identification. This can be a drivers license, a passport, or state issued ID. Next, you must bring in your confirmation letter in hand or make sure any emails sent to you are forwarded to us. We will not be able to fulfill a screening without the precise information needed to perform which specific service needed. Make sure to bring a CHAIN OF CUSTODY if provided to you. Lastly, show up with a full bladder ready to provide a sample once on site. If it is a Hair follicle test, DO NOT SHAVE YOUR HEAD. This will not help you in any way.


HOW DO I PAY FOR MY VISIT?

The quickest, most convenient way to pay for your visit is through our payment portal provided on the scheduling app after you make an appointment. Paying ahead of time will prevent you the hassle of receiving bills via mail and email, and will ensure the easiest method of the transaction. You can also pay in person at the office via card, cash, or check.


HOW DO I KNOW IF IT IS A DOT SCREENING?

A typical DOT screen will be for those in possession of a COMMERCIAL DRIVERS LICENSE or any one with a safety sensitive position in the workplace. Again your ordering official should make it clear via paperwork which test is needed to fulfill your screening.

ANY SPECIAL INSTRUCTIONS FOR A PHYSICAL?

There are certain instances we will need to know of personal information prior to a physical. If you are diabetic, we need your current ALC. If you use a CPAP or BIPAP machine we must have a printed use report. Lastly, if you are prescribed any medications, we need the full list along with your dosages.


WHAT IF I HAVE AN AFTER HOURS EMERGENCY?

In the event of an after hours emergency, please call our office and follow the directions on the pre recording on the phone. An after hours emergency would be if you needed a post accident drug or alcohol screen, if you have court the next day and need immediate proof of screening results, or another instance in which you are willing to get a drug test done at a nominal mark up fee.
